Ethylenediaminetetraacetic acid (abbreviated as EDTA) is an amino polyacid. Because it can form complexes with most metal ions, it has become a general-purpose strong complexing agent. Despite the synergistic effect of EDTA, such as extraction, cation exchange, separation of rare elements, volumetric analysis of calcium, magnesium, barium, softening of hard water, and use as masking agent, metal cleaning agent, antidote for heavy metal poisoning, bactericide have been realized. It has been applied in many aspects, but it is still 1958 years later to make more in-depth basic research on its structure and synergy mechanism.
EDTA salt can protect the cellular components of blood, does not affect the count and size of white blood cells, has minimal effect on the morphology of red blood cells, and can inhibit the aggregation of platelets, which is suitable for general hematological tests. Except for the separation test of platelets, it is not suitable for coagulation test and platelet kinetic energy test, nor for calcium ion, potassium ion, sodium ion, iron ion, alkaline phosphatase, creatine kinase and leucine aminopeptidase measurement and PCR test.
【Precautions】
1. The key to the preparation of whole blood test specimens is complete anticoagulation, so it must be reversed 5 to 8 times as soon as possible after blood collection to prevent the formation of tiny blood clots. Especially when the temperature of the blood collection environment is higher than 25 ℃, the mixing of blood and EDTA tripotassium must be timely and sufficient, otherwise it will easily lead to blood coagulation or local coagulation.
2. EDTA salt anticoagulation is non-irreversible anticoagulation, so the test should be completed within 24 hours after blood collection of EDTA anticoagulation test tube, otherwise it may cause errors in test results. The storage time of EDTA salt anticoagulant specimens is as follows: the white blood cell count can be stored for 48 hours at 4°C, and no more than 24 hours at 23°C. Platelet counts can be stored at 4°C for 24 hours.
3. Anticoagulation requires a sufficient amount of EDTA, but excessive EDTA may cause changes in blood cell morphology, and it is particularly important to accurately control the amount of EDTA tripotassium added.
4. EDTA salts can inhibit platelet aggregation, but cannot completely prevent platelet aggregation. In a small number of patients, platelets are lower than normal in the blood routine test, which may be caused by pseudoaggregation of platelets. In this case, sodium citrate anticoagulant can be used for platelet test verification. This situation has nothing to do with the quality of EDTA anticoagulant, but has a greater relationship with the patient's blood characteristics and clinical testing operations.
5. Tripotassium EDTA can be used in combination with sodium fluoride and separating gel. When using tripotassium EDTA/sodium fluoride, add 1.2-2.0 mg of tripotassium and 2-4 mg of sodium fluoride per milliliter of blood. When EDTA tripotassium/separating gel is mixed, the additive amount of tripotassium remains unchanged, and the amount of separating gel added to each test tube can be 0.8-1.2g. When used with separation gel, the anticoagulation effect, silicification of tube wall, centrifugation conditions, and the quality of separation gel will affect the blood separation effect. It is recommended to use in conjunction with the separation gel produced by our company to obtain high-quality plasma samples.
6. Irradiation sterilization suggestion after adding to the blood collection tube: use γ-ray irradiation, the dose is 8-25kGy. The irradiation dose can be determined from the initial colony count.
The International Committee for Standardization of Hematology (icsh) recommends EDTA tripotassium salt as the preferred anticoagulant for blood count and classification. Compared with dipotassium EDTA, it has higher solubility and faster anticoagulation speed.
